Michael Jai White stars as the eponymous antihero in this supernatural action thriller directed by Mark A.Z. Dippé. After he is betrayed and murdered while on a mission in North Korea, Force Reconnaissance Marine Lieutenant Colonel Al Simmons (White) is returned to life after making a Faustian pact with Malebolgia (voice of Frank Welker), a ruler of Hell.
Simmons agrees to lead Hell's army against mankind as long as he can see his wife Wanda (Theresa Randle) again. Reincarnated as Spawn, he returns to Earth but discovers five years have passed and Wanda has remarried. When he meets Cogliostro (Nicol Williamson), a former Hellspawn who now fights on the side of Heaven, Spawn must decide if he will use his powers for good or evil.
The cast also includes Martin Sheen, John Leguizamo and Miko Hughes.
